Short Breaks Services and Social Inclusion

Children and young people with disabilities should have access to support that ensures that they can participate in the same opportunities as any other child, whilst remaining in their home and community (e.g. through provision of short breaks and support services).

An assessment should be carried out in order to assess the child and carer's needs and provide an appropriate level of service to meet those needs.The assessment will be reviewed every 12 months, or more frequently if circumstances of the child and/or carer change. Folllowing the completion of the assessment and the placement panel meeting an appropriate package of care will be allocated.
