Transition to Adulthood

The Committee promotes joint assessment and planning to enable the successful and seamless transition of a disabled young person from school to adulthood.

Transition in this case means the process of planning and handing over the case of a young person from children's services to Adults with a Disability & Supported Employment Service.The Children's Disability Team will work in partnership with the Education Department to facilitate transition with all/or Child Development Service children who meet the team's eligibility criteria.The formal transition period starts at year 9.  

The Children's Disability Team will liaise with the Adults with a Disability & Supported Employment Service and other Services or the Child Development Service in enough time to ensure that transition will be seamless.
