Transfer of Cases
Children with disabilities who appear to meet the appropriate criteria should be referred directly to the Children's Disability Team with appropriate health and education assessments, if appropriate.
Child protection concerns
Where there are immediate child protection concerns and the child is not an open case to the Children's Disability Team the child should be referred to the Multi-Agency Support Hub (MASH), please refer to Worried about a child? .
When appropriate the Children's Disability Team social worker will offer specific disability advice and support.
Transition to adult's services
Children with Disabilities will be transferred to the Adults with a Disability & Supported Employment Service on their 18th Birthday if they require continuing services.
Children with Disabilities who are "Looked After" will be referred to the Adults with a Disability & Supported Employment Services with support from the Leaving Care Team.
